Who is IPC, Healthcare Inc.?
IPC, inpatient Consultants, is a private medical practice of acute care providers dedicated to the care of your hospitalized patients allowing you to care for and build your office practice. IPC hospitalists refer your patient back to your practice for their follow-up visits.
IPC Hospitalists are:
Well versed in hospital medicine, available 24 hours 7 days a week to cover your hospital calls and are able to admit your patients at facilities where you do not have privileges.
How do our Hospitalists Communicate with you?
Within 24 hours of a patient being admitted to IPC’s service you will receive an Admit Notification that will include the patient’s diagnoses, studies and any consults. Upon a patient being discharged a Discharge Notification will be sent to you including diagnoses, studies, consults, medications, home health and follow-up instructions. Throughout your patients stay, Important patient status and clinical information will automatically be faxed to your office. In addition, upon discharge, IPC’s Nursing Call Center attempts to contact patients discharged to home within 48 hours to ensure proper follow-up with discharge orders and the appropriate return to your office.
How do you refer your patients to IPC?
If you are contacted by a hospital emergency department, you can ask them to admit to IPC. We also can provide you with an “Authorization to Admit to IPC” form for you to sign and fax to the hospital that states you would like patients of yours to be admitted to IPC.
WOUND CARE
IPC’s wound care team is comprised of highly skilled and trained providers that provide comprehensive wound care treatment and management in the post-acute setting along with training and education to staff, patients and families.
Offering weekly scheduled visits, your staff will feel more comfortable caring for residents with wounds and your admissions department will be able to admit more patients knowing that there is a wound care provider to care for your residents with wounds.

RESPIRATORY THERAPY
IPC offers Licensed Respiratory Therapists to assist with respiratory care in the post-acute care setting. Our respiratory therapists will ensure proper care for your patients with respiratory needs including but not limited to: CPAP, bipap and Tracheostomies.
IPC Respiratory Therapists offer:
• In-Services for your staff
• Education for your residents
• Tracheostomy decannulation
• Capping
• Tube changing
• Equipment and medication cost reduction
• Recommendations in Plan of Correction

LONG TERM ACUTE CARE
The long term acute care facilities associated with IPC resource intensive care through our staff of providers. Each patient receives individualized care according to their needs.
LTAC patients need a special kind of facility to receive the resources and time necessary for improvement. Our IPC long term acute care facilities are designed to treat seriously ill patients who require an extended, more attentive hospital stay. To care for this special type of patient, we have teams of doctors, nurse practitioners, and physician assistants dedicated to improving the health of your loved one.
SKILLED NURSING AND LONG TERM CARE

IPC providers give our post-acute care patients high-level quality care while reducing unplanned discharges, which yields a low rate of unnecessary and disruptive hospital readmissions. IPC offers a continuum of care between our providers in the acute setting and those in the post-acute setting. The IPC model provides top quality care for patients which results in a high referral rate coupled with a low unplanned discharge rate.
Provider Availability for Medically Necessary Visits
IPC believes that having a provider available in a facility up to seven days a week directly affects unplanned discharges. Provider presence equals less unplanned discharges.
Medication Management
IPC providers carefully audit their patient’s medications, lowering the amount of medications when applicable. IPC believes patients should receive only amount of medications hey need avoiding dangerous drug interactions.
Care Coordination
IPC care coordinators work directly with facility discharge planners, ensuring a continuum of care for the patient. The overriding goal is to improve the quality of patient care and patient satisfaction.
ASSISTED AND INDEPENDENT LIVING
IPC Physicians, Nurse Practitioners and Physician Assistants (known as Providers) bring medical care to you in your home in assisted and independent living facilities.
IPC Providers are available on call 24-hours 7-days a week for all urgent medical needs. If you or a family member has a question or are in need of a provider visit, IPC offers a 24 hour voicemail line.
IPC works with diagnostic companies that will provide diagnostic services including x-rays, ultrasounds and labs in your home for convenience and comfort.

IPC serving you in Michigan, Ohio, West-Central New York, Kentucky. IPC Michigan Region is part of a large network of physician services under the umbrella of IPC Corporate. Founded by physicians in 1995, IPC is the nation’s leading private practice hospitalist group.
